Welcome to the Martin Health Center at Taft!
After reviewing the requirements below, please log in to Magnus to access your child's health forms.
Important Dates:
Five week program: Health forms are due June 1st, 2026
Two week program: Health forms are due June 15th, 2026
THE FOLLOWING FORMS ARE REQUIRED BEFORE YOUR CHILD ARRIVES ON CAMPUS:
Vital Health Record (medical history and demographics) — Digital form
Physical Exam — Must be dated after July 1st, 2025 and signed by an MD, APRN or PA. Must include a negative tuberculosis skin test OR Quantiferon Gold blood test within the six months prior to arrival at Taft.
Immunization Record — A copy of your child's complete vaccine record, including dates of immunization, signed by MD, APRN, or PA.
Insurance Card — All students must have active, US-based medical insurance for the duration of the Summer@Taft program.
- Domestic students: Upload a copy of front and back of insurance card.
- International students: Required to purchase international insurance during the enrollment process. No upload needed.
